ESMO Breast 2025 | The role of CDK4/6 inhibitors with ET in HR+/HER2− breast cancer

Christian Singer, MD, Medical University of Vienna, Vienna, Austria discusses the role of CDK4/6 inhibitors in combination with endocrine therapy (ET) for hormone receptor-positive, HER2-negative (HR+/HER2−) breast cancer. He confirms this combination as a well-established first-line option in the advanced setting, using either aromatase inhibitors or fulvestrant. As these agents move into earlier disease stages for high- and intermediate-risk patients, their role upon progression may be influenced by the disease-free interval. In rapidly progressing cases, alternative or more complex treatment strategies, including possible reintroduction of CDK4/6 inhibitors, may be considered. This interview took place at the 2025 European Society for Medical Oncology (ESMO) Breast Cancer Annual Congress in Munich, Germany.
