ESMO 2026 | Cost-effectiveness of first-line chemo-immunotherapy in solid tumours

Jacopo Canzian, MD, Humanitas University, Rozzano, Italy, discusses a systematic review of cost-effectiveness analyses of first-line immune checkpoint inhibitors combined with chemotherapy versus chemotherapy alone in advanced solid tumours. The combination was rarely cost-effective in high-income countries and almost never in lower- and middle-income settings, regardless of clinical benefit. Chemotherapy followed by immunotherapy (IO) maintenance appeared the most economically favourable strategy. This interview took place at the European Society for Medical Oncology (ESMO) 2026 Congress in Madrid, Spain.
