Icro Meattini, MD, University of Florence, Florence, Italy, discusses the current evidence supporting stereotactic radiosurgery (SRS) as the preferred treatment for brain metastases in breast cancer. SRS has shown effectiveness even in patients with multiple lesions and whole brain radiotherapy (WBRT) is now reserved for select cases. Patient selection for SRS should also be guided by disease biology, treatment history, comorbidities, and overall fitness, and that for most eligible patients, SRS should be the first-line radiotherapeutic option. This interview took place at the 2025 European Society for Medical Oncology (ESMO) Breast Cancer Annual Congress in Munich, Germany.
