Johannes Holtschmidt, MD, German Breast Group, Neu-Isenburg, Germany, provides an overview of the challenges in accurately identifying residual node-positive breast cancer patients after neoadjuvant systemic treatment. Despite 20–30% of patients being node-positive at baseline, fewer than 3% remain so after treatment, yet current imaging lacks reliability in pinpointing these individuals. Subsequently, there is a need for improved detection methods to accurately stratify patients and guide therapeutic decisions, including adjuvant pertuzumab, to optimize outcomes for high-risk groups. This interview took place at the 2024 San Antonio Breast Cancer Symposium (SABCS) congress in San Antonio, TX.
