ESMO 2024 | Evaluating neoadjuvant FLOT response and the need for biomarker-guided strategies in GEA

Radka Obermannova, MD, PhD, Masaryk University, Brno, Czech Republic, emphasizes that the findings from a real-world study (ACTRN12622000180718) in gastroesophageal adenocarcinoma (GEA) are not practice-changing due to limitations such as non-unified tumor regression grade (TRG) evaluation and differences in baseline characteristics. Despite its ambition, some valuable treatment benefits may have been missed. For patients not responding to neoadjuvant FLOT, more biomarker-driven strategies are needed. Biomarker-guided algorithms, like PET scans, may help predict non-responders and guide treatment adjustments before surgery, especially as targeted therapies and immunotherapies in adjuvant settings have faced challenges. This interview took place at the European Society for Medical Oncology (ESMO) 2024 Congress in Barcelona, Spain.
