ESMO Asia 2025 | Perioperative durvalumab plus FLOT improves outcomes in gastric cancer

Ken Kato, MD, PhD, National Cancer Center Hospital East, Tokyo, Japan, provides an overview of recent developments in gastric cancer, especially in Asian patients. In the MATTERHORN study (NCT04592913), which showed the superiority of perioperative durvalumab plus FLOT in terms of event-free survival and overall survival, the Asian subgroup presenting a better survival rate compared to the Western population. This interview took place at 2025 European Society for Medical Oncology (ESMO) Immuno-Oncology Congress in London, UK.

Yeah, of course, yeah, so uh I’m also in charge of the gastric cancer development, so I focus on the uh also the perioperative chemotherapy, so already the result of the MATTNHORN study was presented in the ASCO and ESMO for the entire population, and it shows the superiority of the population of event-free survival and its overall survival. This ESMO-Asia meeting, the Asian subgroup results were presented by the Arjan region. So for the former studies, totally the Asian population shows a better survival compared to the Western population. And then the magnitude of the new drug is less than the Western population. I uh focused on the so the baseline uh survival outcome of the Asian population and hazards ratio of the so default uh arms, and so the result we show the similar uh pathological complete response rate compared to the global population and Asian population. And from the Asian, only 20% of the patients were enrolled from the Asian. But the pathological complete response rate is similar to the ITT population. As for the event-free survival, the two-year event-free survival rate for the plus-n-plus-fluoropyrimidine regimen is 5% more in the ASEAN population compared to the global population. But the hazard ratio is similar between the Asian population and the global population. That means the deliverables show the additional benefits, even for the Asian patients who had a better survival, eventually survival, than the global population, and for the overall survival also, the three-year OS rate is so 10 percent higher in the Asian population compared to the so global population, and the three years of our response of OS rate for the placebo fluoropyrimidine is 62% in the global population and 69% of the D-fluoropyrimidine regimen. But in the Asian subgroup, 72% for the placebo fluoropyrimidine of the three years survival rate, but 76% plus the 4% of the three-year survival rate, but it’s 76% plus the 4% of the three-year OS rate. And the hazard ratio is relatively better with the Asian subgroups than the global population. So I think the efficacy of the Durvalumab is very robust even for the Asian population. And the Japanese subgroup result will be held in the JASMO meeting held in the Japanese gastric association held in March. And I expect all the results of the Japanese subgroup of the Matterhorn study.
