ESMO Breast 2025 | Post-progression CDK4/6 inhibitors in breast cancer: a failing strategy?

Fabrice André, MD, PhD, Gustave Roussy, Villejuif, France, shares his rationale against the continuation of CDK4/6 inhibitors following disease progression in patients with hormone receptor-positive, HER2-negative breast cancer. Much of the available data, particularly supporting continued use, is based on abemaciclib, which differs significantly from palbociclib. As such, what is often presented as “continuation” may actually reflect a switch in agents, not a true continuation. Moreover, trials involving ongoing use of the same CDK4/6 inhibitor have shown minimal, if any, clinical benefit. Prof. André emphasizes it is not sufficient to be considered a meaningful or transformative option for patients, even when modest benefit is observed. This interview took place at the 2025 European Society for Medical Oncology (ESMO) Breast Cancer Annual Congress in Munich, Germany.
