ESMO 2025 | Patient-derived organoids guide personalized therapy in thyroid cancer

Mauro Cives, MD, University of Bari Aldo Moro, Bari, Italy, highlights the use of patient-derived organoids (PDOs) to predict clinical response and guide personalized therapy in advanced thyroid cancer from a Phase II study (NCT06482086). PDOs were successfully established from most patient samples and recapitulated tumor histology and genomics. Drug screening on PDOs accurately predicted patient response, and PDO-guided therapy achieved high disease control and objective response rates, including enabling surgical resection in selected cases. This interview took place at the European Society for Medical Oncology (ESMO) 2025 Congress in Berlin, Germany.

This is a very interesting study coming from China describing how we can, let’s say, go to the next level in treating cancer patients by using a personalized medicine approach. Colleagues from China derived organoids from patients with thyroid cancers and then tested different drugs and then of interest they treated these patients with the drugs that in vitro provided the best results and by doing so they were able to demonstrate that patients could benefit from the treatment so you know like in the future maybe there is a chance for us to improve our treatment modalities for patients with thyroid cancer of course this is not a randomized trial it would be extremely important in the future to compare these data with data driven by genomic stratification of these tumors...

In other terms, what is the impact of the organoid generation on the treatment outcomes instead of the genomic background of the tumors per se.
