ESMO GI 2025 | PROMETICO presents novel insights into metastatic colorectal cancer

Rocio Garcia-Carbonero, MD, PhD, University Hospital 12 De Octubre, Madrid, Spain presents findings from PROMETICO (NCT03935763), the first international prospective real world-study evaluating patients with metastatic colorectal cancer after two disease progressions, including patients with ECOG performance status (PS) of 2, a subgroup typically excluded from clinical trials. TAS-102 was the most frequently used regimen, and higher adverse event rates, such as anemia, reflected more advanced disease progression. This interview took place at the European Society for Medical Oncology (ESMO) Gastrointestinal Cancer 2025 Congress in Barcelona, Spain.

Yes, well the PROMETICO study is the first international prospective real-world data on patients with metastatic colorectal cancer that have experienced two disease progressions and are about to initiate a third-line treatment. So these patients, about over 700 patients, we enrolled in PROMETICO trial. And we have assessed in these patients a number of things, including quality of life assessments. Here at ESMO GI, we have analyzed safety profile according to performance status before PROMETICO study entry. And so what we find is in this over 700 patients about the most frequently used regimen in this third line setting was TAS 102. That is well because patients were enrolled also before the Sunlight trial results were available. And 90% had a performance of 0 to 1, but there was a 10% patient population with performance status of 2, which are generally excluded from clinical trials. So we analyze safety in this group of performance status 2 and also patient characteristics. So we observe this subgroup of patients with a poor performance status. They have generally or older age, more right-sided tumors, higher number of metastatic disease and higher in a rate of liver metastases. So generally speaking, more advanced or poor prognosis as expected. And so what we see in these patients, they experience a higher rate of adverse events, primarily of anemia, probably reflecting this more advanced disease and also a high rate of severe adverse events like in a 10% greater rate. But other than that, you know, outcomes are similar. And the most common adverse events in this patient population is first disease progression, also neutropenia, anemia, asthenia or fatigue, nausea and diarrhea.
