VJVirtual | From cold tumors to hot targets: immunotherapy advances in sarcoma treatment
Javier Martin-Broto, MD, PhD, Fundación Jiménez Díaz University Hospital, Madrid, Spain, discusses the nuanced role of immunotherapy in sarcomas. While most sarcomas are characterized by a low tumor mutation burden (~2 mutations/Mb) and a cold tumor microenvironment, select subtypes exhibit notable immunosensitivity. Alveolar soft part sarcoma (ASPS), for example, responds well to anti-PD-1 agents like tislelizumab, despite lacking classical biomarkers of response. Emerging data on immunogenic cell death, particularly with doxorubicin in leiomyosarcoma and undifferentiated pleomorphic sarcoma, support combination strategies with checkpoint inhibitors. Additionally, TCR-T cell therapy is showing promise in synovial sarcoma and myxofibrosarcoma. These findings highlight immunotherapy’s evolving niche within sarcoma treatment paradigms.
