Kevin Kalinsky, MD, MS, Winship Cancer Institute of Emory University, Atlanta, GA, reviews the clinical utility of genomic assays in HR-positive, HER2-negative early-stage breast cancer, including the 21-gene and 70-gene assays, with emphasis on recent findings from the OPTIMA study (ISRCTN42400492). The trial required patients to be at least 40 years of age and showed that ovarian suppression plus endocrine therapy for premenopausal N1 disease was not superior to chemotherapy followed by endocrine therapy, addressing a key clinical question in the field. Future developments in this area are likely to incorporate artificial intelligence and pathomics approaches. This interview took place at the European Society for Medical Oncology (ESMO) 2026 Congress in Madrid, Spain.
