ESMO Asia 2025 | mRNA expression as a predictor of TKI efficacy in advanced solid tumors

Takao Fujisawa, MD, PhD, National Cancer Center Hospital East, Kashiwa, Japan, discusses the SCRUM-Japan MONSTAR-SCREEN-2 study (UMIN000043899), which evaluated whether mRNA expression of tyrosine kinase inhibitors (TKIs) target genes predicts response in advanced solid tumors. High mRNA expression was associated with significantly improved objective response rate and progression-free survival in patients treated with TKI monotherapy, including VEGFR-TKIs. These findings suggest mRNA expression may serve as a pan-TKI predictive biomarker, independent of specific driver alterations or TKI targets. This interview took place at 2025 European Society for Medical Oncology (ESMO) Asia Congress in Singapore, Singapore.

Okay, thank you. Let me introduce our project, MONSTAR-SCREEN-2. Yes, Scrum Japan Monster Screen is the translation research target for the patient with advanced tumors. And in this study, we analyzed some genomic analysis, including the tissue whole exome sequencing and tissue whole transcriptome sequencing and the blood-based whole exome sequencing and whole transcriptome sequencing was performed. Regarding the tissue and the blood whole exome NGS testing are performed by the Claris Life Sciences in US. And we analyzed the tissue and the blood NGS testing before initiation of the systemic therapy and after progression of systemic therapy. And we would like to detect the molecular targets of the patients and evaluate the molecular resistance mechanism to analyze the tissue samples after the progression after the disease progression.
