Maria Vittoria Dieci, MD, PhD, University of Padova, Padova, Italy, discusses the findings of a study examining tumor-infiltrating lymphocytes (TILs) as a prognostic biomarker in HER2-positive breast cancer within a treatment de-escalation trial. The analysis revealed, for the first time, that high levels of TILs are associated with improved overall survival (OS) in patients receiving adjuvant therapy, allowing for safe treatment de-escalation without increasing the risk of distant relapse or death. Dr. Dieci highlights the importance of integrating TILs with other prognostic factors, such as disease stage, and mentions the potential role of genomic tests like HER2DX, which combine clinical features and immune-related signatures to guide personalized de-escalation strategies.
