Walter Weber, MD, Basel University Hospital, Basel, Switzerland, comments on the role of the sentinel lymph node biopsy in the upfront surgery setting. Results from the SENOMAC (NCT02240472), INSEMA (NCT02466737), and SOUND (NCT02167490) trials show that sentinel lymph node biopsy can be safely omitted in selected patients with small tumors and negative ultrasound findings. This approach may be reserved for younger patients or those with more aggressive tumor characteristics, such as stage 3 cancer, HER2-positive, or triple negative breast cancer.
