WCLC 2025 | The evolving role of datopotamab deruxtecan in lung cancer

Sewanti Limaye, MBBS, MD, MS, Sir H. N. Reliance Foundation Hospital & Research Centre, Mumbai, India, comments on the application of datopotamab deruxtecan (Dato-DXd) in non-small cell (NSCLC) and squamous lung cancer. While data from the TROPION-Lung01 (NCT04656652) and TROPION-Lung05 (NCT04484142) trials support its use in NSCLC, emerging evidence also suggests its efficacy in squamous lung cancer, albeit not as a first-line option. Currently, Dato-DXd is being administered to all comers in NSCLC, regardless of biomarker expression, due to its observed benefits in both high and low expressors. This interview took place at 2025 World Conference on Lung Cancer (WCLC) in Barcelona, Spain.

How does data apply for non-small cell and squamous? Now we have application of Datopotamab deruxtecan in both non-small cell and squamous. So non-small cell, adenocarcinoma and squamous. And we know largely the application and the benefit is better described in non-squamous, non-small cell. But emerging data also in squamous. Of course, not the first option, but really right now the application of Datopotamab deruxtecan in non-squamous, non-small cell lung cancer is without biomarker specific application because the benefit is being seen in all comers...

Of course, not the first option, but really right now the application of Datopotamab deruxtecan in non-squamous, non-small cell lung cancer is without biomarker specific application because the benefit is being seen in all comers. It is a biomarker that’s seen expressed in most non-small cell lung cancers and in the TROPION-1 and TROPION-5 lung trials, the enhanced benefit was seen in people who were higher expressors, but the benefit was seen nevertheless. So right now for Datopotamab deruxtecan, we are applying in all comers.
